All Alaska Football Camp (4 Days – West Valley High School) Online Registration

Must Read – Athletes cannot participate in the camp without the following: 1. Copy of physical examination with physician’s signature dated after December 31, 2021. Must be on file with Camp Staff no later than registration day. Physicals will be returned at the end of camp. 2. Current copy of medical insurance coverage. Must be on file with camp staff no later than registration day.

My son has my permission to attend the All Alaska Football Camp LLC. I hereby authorize the staff of All Alaska Football Camp LLC to act for me according to their best judgment in any emergency medical situation. I hereby waive, release, exonerate, and discharge the All Alaska Football Camp LLC, its employees, and volunteers from any or all actions causes known or unknown, from any injuries or illness incurred in Camp or on the way to or from Camp. Costs for treatment of injuries and hospitalization for illness or injuries incurred during the sports camp will be the responsibility of the parent or guardian of the participant. Any insurance carried by the parent or guardian may be used to defray such medical and hospital costs. I certify that my child has no injury or illness, which would limit his participation in Camp.

I understand by signing this application that I will comply with the rules of conduct set down by All Alaska Football Camp LLC. I agree to meet All Alaska Football Camp’s expectations of behavior in order to help establish and enhance an atmosphere of positive learning. I also agree to adhere to rules set forth by the School District as they pertain to their schools and educational system. I understand that violators will be sent home at parent’s expense.

I am the minor’s parent or guardian named above and I understand the nature of the Waiver of Liability above and verify and consent to the minor attending the All Alaska Football Camp. On the minor’s behalf, I hereby release, covenant not to sue, discharge, and hold harmless the All Alaska Camp, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. On behalf of the minor, I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the All Alaska Football Camp, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending in any All Alaska Football Camp program or activity.

No Refunds after June 10th. All refunds will be minus any online convenience fee.
